Ask the provider to include these answers in one quotation appendix. Unpriced items must be marked excluded or pending confirmation, not counted as zero. Additional testing such as PGT-A should be listed only as applicable to the individual clinical decision.

Article 104 of the Citizen Health Protection Law is a starting point, but it does not by itself prove that every foreign intended parent can proceed. Independent local counsel, the clinic and the notary should confirm current implementation, identity and medical conditions, contract effect, parentage, birth registration and return-home documents in writing.
Marital or partner status, medical conditions, clinic policy, contracts, parentage and birth registration should be checked under current law and practice. Nationality, travel documents and registration in the family's destination country require separate review.
States differ on contract enforcement, parentage procedure, eligibility and birth documents. Independent counsel in the programme state should confirm applicable law, the parentage order or judgment, insurance and hospital documents; nationality and return-home registration require separate advice in the destination jurisdiction.
