Keep the two women’s prenatal records and delivery papers in separate files. Preserve original dates and providers on blood-pressure and ultrasound records, with signed papers filed separately. Each treating team confirms follow-up visits, admission contacts and papers to bring, allowing the two pathways to be coordinated individually.
